To reduce the transmission of covid-19 and for your convenience, we’ll be offering you the ability to monitor your condition. To do this, use the boxes below to ensure that:

  • You’re confident in using your inhaler
  • You complete an asthma action plan– if you would like us to print one for you, please get in touch
  • You complete the asthma control test. For your convenience we’ll send you a text message with this included.

If you’re happy with your asthma control and if your asthma control answers are satisfactory, we’ll review your medication list. If at any point you feel that you’d rather speak to a clinician about your asthma monitoring, just let us know by contacting reception.

Consider asking a friend or relative to go through this with you-they may sometimes think of something that you may not have realised. If you or someone you know are unable to use our website you will be given a paper version of this information- just let us know if this is the case.
