How to register

We are pleased to accept new NHS patients who live within the practice boundary illustrated on the map below. The practice provides general medical services for Bilbrook and Codsall. We also have patients registered in Perton,  Pendeford,  Tettenhall and Ford houses.

It’s simple to join us. You can now register online via this link, by completing the forms below and submitting them to reception or by collecting and completing a registration pack available at reception.

You don’t need proof of ID, but it would be helpful if you have any of the following:-

 

  • Your NHS number (you may have this on any hospital/GP letter)
  • Proof of Address i.e. any household utility bill, council tax bill or a recent bank statement.
  • passport
  • Birth Certificate
  • HC2 Certificate
  • Rough sleepers’ identity badge
  • Hostel or accommodation
  • If you are homeless you can give a temporary address, such as a friend’s address or a day centre.

It is important that you inform reception of any changes  of address or contact details.

Then simply hand the completed forms in at reception. When we have received your medical records we may write to you and request that you attend the surgery for a new patient check to discuss your medical history.

Patients moving out of the practice area

When a patient moves out of the area they will be asked to register at a practice nearer to where they live. Where a patient wishes to stay with the practice please write to the Practice Manager for consideration as an Out of Area with No Home Visits. Considerations are based on whether it is is clinically appropriate or practical on an individual basis.

Patients with particular needs

The surgery is accessible to patients using a wheelchair, providing disabled parking and toilet facilities. To ensure the safety of our patients, large mobility scooters are to be left in the entrance porch. All clinical facilities are on the ground floor. For patients with hearing problems, a loop system is installed in the reception area. We can arrange interpretation and translation services for patients who do not speak English. Please let us know if you need this service when booking an appointment.

‘Ghost Patients’

NHS Digital is in the process of finalising plans to check Doctors patient registers to identify ‘ghost patients’. These are patients who appear to be registered at more than one practice.  They will be writing to all patients not seen by their GP for 5 years or more, asking them to confirm to their practice that they are still living in the area. Where a patient does not contact the practice NHS Digital will assume they are no longer registered.
